In general, PMS symptoms start around one week before menstruation and typically go away once menstruation begins.

Causes
The cause of PMS is unknown. But a change in both sex hormones and serotonin levels at the beginning of the menstrual cycle is believed to play a role in PMS by experts.
Physiopathologie
Premenstrual syndrome (PMS) and premenstrual dysphoric disorder are triggered by hormonal events ensuing after ovulation. Although evidence for a hormonal abnormality has not been established, the presenting complaints of the condition can be related to the production of progesterone by the ovary. The lowering of serotonin also can give rise to PMS-like symptoms. However, similar to affective disorders, PMS is not completely related to the dysregulation of any neurotransmitters.
Diagnostic
There is no single examination or lab test to diagnose the premenstrual syndrome.
Traitements
For many women, lifestyle changes can help relieve PMS symptoms. But in severe cases, medications are needed. The success rate of medications varies among women. Most advised medications for premenstrual syndrome include:
- Selective serotonin reuptake inhibitors (SSRIs) — which include fluoxetine (Prozac, Sarafem), paroxetine (Paxil, Pexeva), sertraline (Zoloft) and others — have been successful in reducing mood symptoms.
- Nonsteroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen (Advil, Motrin IB, others) or naproxen sodium (Aleve) can ease cramping and breast discomfort.
- Diuretics like Spironolactone (Aldactone) can help ease some of the symptoms of PMS.
- Hormonal contraceptives that stop ovulation, can give relief from PMS symptoms.
Pronostic
PMS has a very wide range of prognoses. It gets better mostly in women with a healthy lifestyle.

If managed properly, no complications may arise.
